Position reports to: HCBS Manager
Job Description: Services designed to assist individuals in acquiring, retaining and improving the self-help, socialization and adaptive skills necessary to reside successfully in home and community-based settings. The services include the provision of training in independent living skills or special developmental skills, orientation and mobility training, sensorimotor development, and behavioral management.

Job Description: Services designed to assist individuals in acquiring, retaining and improving the self-help, socialization and adaptive skills necessary to reside successfully in home and community-based settings. The services include the provision of training in independent living skills or special developmental skills, orientation and mobility training, sensorimotor development, and behavioral management.
Essential Job Functions:
- As identified in the consumer’s ISP, provide training and/or assistance such as:
Assistance and training related to personal and physical needs and routine daily living skills; Implementing strategies to address behavioral concerns, developing behavioral intervention programs, and coordinating with behavioral health program to ensure proper review of medication treatment plans; Ensuring that the health needs of the consumer are being met, including providing follow up as requested by the consumer’s primary care physician or medical specialist; Implementing all therapeutic recommendations including speech, occupational, and physical therapy and assisting consumers in following special diets, exercise routines, or other therapeutic regimes; Mobility training, alternative or adaptive communication training; Opportunities for training and/or practice in basic consumer skills such as shopping, banking, money management, access and use of community resources, and community survival skills, etc.; and Assisting consumers in utilizing community transportation resources to support the consumer in all daily living activities, e.g., day treatment and training, employment situation, medical appointments, visits with family and friends and other community activities, etc., as identified within the consumer’s ISP. - As identified in the consumer’s ISP, provide training and/or assistance to the consumer’s family/consumer’s representative to increase and/or maintain targeted skills acquisition of the consumer.
- 18 years of age
- Documentation showing that fingerprints have been taken
- A completed declaration of criminal history
- Maintains and demonstrates competence in the field of developmental disabilities.
- A current CPR certificate
- A current First Aid certificate
- A current Article 9 certificate
- Client intervention techniques training if indicated in the ISPP or requested by the parent or guardian
- Such other training as indicated in the ISP
- When providing transportation services the employee shall maintain a current valid driver's license, valid registration, and current liability insurance coverage
- Three letters of reference who are not the applicant's family members and who have personal knowledge about the applicant's employment history, education, or character
- Have at least three (3) months experience implementing and documenting performance in individual programs (specific training strategies)
- Have both three (3) months experience in providing either respite or personal care and have received training, approved by the Division, in implementing and documenting performance or perform three (3) months of habilitation services under the direct supervision of an individual who is qualified to provide habilitation as described above
